Applicant Certification & Authorization
I certify that the information provided in this application is true and complete to the best of my knowledge. I understand that misrepresentation or omission of facts is cause for dismissal. I authorize the company to make inquiries regarding my driving record, employment history, criminal background, and any other investigation deemed necessary. I understand that employment is contingent upon satisfactory completion of all required background checks, drug testing, and medical certification. I authorize my previous employers to release information regarding my employment to the company. I understand and agree that my employment is "at will" and can be terminated by either party at any time for any reason.
Disclosure & Consent for Background Investigation
In connection with my application for employment with the company, I understand that a consumer report and/or investigative consumer report may be requested. This may include information regarding my character, general reputation, personal characteristics, mode of living, criminal history, motor vehicle record, and employment history. I hereby authorize the company and its designated agents to obtain such reports and to make any investigation of my background as may be necessary.
Probationary Period & Pre-Employment Cost Reimbursement
I understand that, as an applicant for a position with the company, I may be asked to demonstrate through trial or student trips or other means, that I am capable of performing tasks that may be pertinent to the job, during my probationary period. During this probationary period any training, whether on the coach or in the classroom, will be paid at the standard hourly rate; however, all wages for said training will be paid to me only upon successful completion of the aforementioned probationary period. I also understand that if offered a job, it may be conditioned upon results of a physical examination, a drug and/or alcohol breath test, results of my motor vehicle record check, criminal background check and child abuse history clearance. I also understand and agree that as a condition of my employment the cost of the aforementioned physical examination and/or drug test will be deducted from my compensation. This cost will be reimbursed to me after I have completed twenty (20) days of paid employment, or twenty (20) trips with the company.
